Expert Health Care

Each resident receives a personalized care assessment from a licensed nurse to assess their individual needs. An individualized plan is then created with your needs in mind.

Nursing services include a licensed nurse on-call 24 hours a day, seven days a week

Medication support and reminders along with ongoing health education in addition to obtaining medication ordered by your health care provider

Monitoring of health status in consultation with the medical professionals involved in your care

Coordination of services and referrals to health-related services as needed

Specialized intervention programs such as nutrition tracking, weight management and falls management
